A Momentum letter dated 11 May 2015 which clarify the reasons why Mrs Shaik's claim was declined, was posted to her. Mrs Shaik may contact Momentum Claims Department directly on ***to request that a copy of the noted letter to be faxed or e-mailed to her as such contact details are not available on her policy records.
