As previously requested to you since December 2018 to contact Mr. Vice urgently via phone or his next of kin Sister- Elaine Jacobs ********** , Brother in Law - Hennie Jacobs ********** if you cannot get hold of him.

Mr. Vice does not have an email address or respond on mail send by post, and does not call back if a voice massage are left, therefor it is important to call him on his cell or his Sister- Elaine Jacobs ********** , Brother in Law - Hennie Jacobs ********** .

I have no contact with him due to a restraining order I have against him and are not allowed to contact him otherwise it will affect my restraining order against him.

If you intend to call me please use the number ********** 454. If this policy are made paid up I will certainly hold Momentum accountable for the loss off R246 000.00 that are part of the agreement and court order regarding the session I have on this policy.

Thank you for your e-mail of 11 January 2019 to enquire about the premium status.

Kindly note the policy is currently in arrears with 2 preMiums and total arrears amounts to R 311.26.

We have informed the premium payer via e-mail but the same e-mail is on record for policy holder and premium payer.

Once the policy is in arrears with 3 premiums the policy will be made paid up and monthly debit order will be stopped.

We understand the situation but we are not allowed to get involved in the dispute between policy holder and premium payer.

You do have the option to change premium payer please refer to attach debit order form. Thank you for interacting with us.
