FNB Pension Fund Surplus - Tax Directive

I app**** for an successfully received a surplus payout in April 2017. Momentum duly sent me an IRP5 for SARS purposes for the 2018 tax year. On completing my Feb 2017 tax return on e-filing I encountered a problem which turned out to be caused by this pension surplus payout. To resolve matters with SARS I am needing a copy of the tax directive and application for tax directive that Momentum would have submitted to SARS to determine the amount of tax that was deducted by Momentum from my payout. I am struggling to get this out of Momentum. I first spoke to a Theresa Harvey ( ********** 433) who promised to come back to me same day, then when I followed up she said the matter had been referred to a Kieran Nadhad ( ********** 560) but his phone goes unanswered.
