Deceased letter of nomination

I am utterly disappointed in Momentum service, THE WORST!!!! For the past 2 years I've been going back and forth with consultants who end up referring me to the same person, Andile Ntuli. When my mother passed away, I requested a letter of nomination that is required by the department of labour to process our claim. Andile Ntuli blantly refused to issue the document telling us about the company's policy. Even worse I'm staying with my baby sister with no financial assistance from anyone and the 18 months claiming period has already expired. WHAT DO I DO NOW?????
