Dear Chenelle

I have tried to call you however you are obviously busy. This claim was originally notified as a withdrawal claim on 11 October 2017 and Momentum entered into various correspondence as this should have been noted as a death claim.

the funeral benefit was paid to yourself on 2 November 2017, Our Portfolio Head Allison also spoke to you regarding the claim.

There are two households involved in this claim and in order for our Trustees to make a fair decision on the resolution of this claim we still require the following information:

Proof of dependency (and value in Rand amount) for Johanna (partner) Simone (daughter) and Chenelle (daughter)

As well as Employment status and confirmation of income for each of you.

Once we have received this information, we will be in a position to submit this to the Trustees in order that they may approve a resolution which will be fair to all parties involved.

Kind Regards

Sue
