COVID-19 Vaccine Breech and Medical Negligence Ethics

I would like to log a complaint for medical negligence and compensation.

During my first Covid-19 vaccine when my information was not log on the system after my first vaccine was given to me by momentum at the Parc Du Cup Mispel Road Belville Cape Town Branch.

This is my personal life and data that got taken for grated.

Something so important was injected inside my body and nothing was registered on the system for the Department of Health neither was a Health official details on who provided the injecton on the 27/07/2021.
