claim declined

I was on permanent health insurance for almost four years due to major depressive disorder. I was better and could return to work last year August. I had a major setback and re-app**** for PHI. I was on sick leave for more than three months. After three and a half months I phoned Momentum and asked why I haven't received feedback yet. I found out that no claims officer had touched my file. After a week and a half, I received feedback that my claim was declined.
