Cancellation of Momentum disability cover

I had a work incident and was medically boarded, I have memory loss due to this incident and could not remember all the dates that Momentum asked me to visit a Dr as I was in the process with Liberty as well. My wife left me and then Momentum cancelled my disability funds monthly and due to that I also lost my medical.
