Need help

<p>Really need assistance in this matter from AF. After I received assistance previously from AF by tracking my provident fund withdrawal forms from my ex employer I am now in a situation where the employer has not submitted my liability (indebtedness) form. I contacted them yesterday after AF informed me of this and they told me the form will be send by end of day yesterday. I really do not believe this as they **** about submitting the withdrawal forms in the 1st place. Really need someone from AF to assist me in this matter as I am starting to lose hope that my claim will ever be paid out two months after leaving this employer.</p>
