Travel insurance request

I've been trying to request my travel insurance coverage. I e spoken to 3 different consultants. They've asked me to fill in a questionnaire and email it back to them before they could issue my travel insurance. I've emailed that form back to them 3 times. Still no response from them. I'm leaving on Monday and I still haven't received my travel insurance and yet I pay my premiums every month. Their behaviour is appalling.
