Momentum Health failing to supply mandatory SARS information

I recently terminated my membership with momentum health due to them not honoring most of my medical claims. In addition to this they started me off on a massive discount for the "same" benefits that I enjoyed at Discovery medical but over a three year period I was systematically increased to well over what I was paying at Discovery. As SARS tax season has now been opened I have tried on numerous occasions to just get my membership tax certificate out of them. As I could not get my tax certificate out of them I went ahead and filed my IRP 5 with the momentum details already been uploaded by my employer. SARS efiling does not offer you the option to remove this pre loaded info. My tax file has now been selected for audit and SARS specifically want to see my medical certificate which I do not have. I spoke to one of the SARS agents to inform them that I am unable to get this certificate from Momentum but was informed by SARS that failing to produce the supporting documentation that I used to file my tax return with is a ******** offence and that SARS may take action against me. In the mean time I am only getting automated email address response from Momentum and on one occasion they sent me a membership change of details form as my email address changed. This despite the fact that I produced all other relevant personal information in my correspondence with them to prove that I am who I say I am. This is so frustrating!!!
