Momentum Covid-19 Vaccine Health Breech and Negligence.

Momentum/Metropolitan in Belville Parc Du Cup business Park. Did not register my details with the Department of Health after I completed a detailed application form. They injected me with my first Phizer Covid-19 vaccine and never registered it. This is a major breech in health protocol and ethics I lodge my complaint for Compensation with Momentum and informed the Department of health. I asked them to send me complaint forms and compensation forms but they have not sent me the forms. They must be held responsible for their negligence around my COVID-19 Vaccine and that it was never registered on the Department of Health system neither was the Health official details provided on who injected in the Momentum building in Bellville Branch. (Parc Du Cap, Mispel Road, Bellville.) They better send me complaint forms and compensation forms. I will take this matter to court and the media if I have to. Department of health in Pretoria and Western Cape has been informed of my complaint and compensation claim against Momentum Parc Du Cap, Mispel Road, Bellville Branch.

I don't want any calls from them regarding my complaint and compensation complaint claim they should email me whatever they would like to tell me I want them to put everything in writing.
