IRP5

Hi Momentum, I’m one of your former employee. I used to work for Metropolitan Health Group back in 2011 and I was there when it merged with Momentum. I wrote an email in August asking for my IRP5. A lady by the name of Sipokazi Pango assisted me. I received my IRP5 but it showed I was on a medical aid provided by the company. I never had medical aid at that time. Now I have been calling and sending emails to her without any help. SARS is going to charge me for something I never had. If you could kindly assist me as soon as possible.
