Thank you for allocating the required attention to my application. However i am in the dark still when it comes to the type of cover that i am getting under this plan.

most of the benefits are offered in state facilities. what is covered in private hospitals so i have come up with a few potential scenarios because i cant find an explanation to this in the brochure.

patient suffering heart attack needs and angiogram and possible 2 stents 5 days icu covered is full yes or no is angiogram/plasty covered

scopes in hospital for pancretitis.

memogram for early detection is it a screening benefit or on the day to day radiology formulary.

the dependents i am adding. the waiting periods on the plan what if they are hospitalized after the 3 months waiting period for an emergency relating and a negative effect of the primary cover that the are having waiting period for say the patient gets blind because of diabetic that is a waiting condition will he be treated for blindness.

the brochure says at state facilities does this mean that momentum has an arrange ment with these facilities are is momentum saying go die heartlessly at any government mortuary cause that is what is the state facilities. can this be considered and exclusion if they say state facilities.

can i be emailed the details for the acute and chronic formulary.

the 270 pmb conditions as per regulation were must treatment be saught as the medical aid suppose to cover this compulsory.

if i register for chronic say for asthma do i get a basket of care as per pmb treatment plan gazetted.

if i choose the any hospital option does it really mean that i can go to any hospital will i be liable for rates charged above or what in heavens name does this mean as it does not state any hospital subject to....
