after 1 year no medical card

I start with Momentum health in September 2014 without receiving a membership card. I requested a card numerous times without receiving it yet. When I visit the doctor I paid for it and the medicine due to it. My monthly premium is deducted every month without getting anything out of it. Is there someone at Momentum that can help me!
