Waiting for provident fund

I submitted my application for my provident fund in November 15 and after 6 weeks I called only to be told the forms were signed for but lost. After numerous emails the forms were couriered and still never received. However when I get through to a consultant I get a different story and first the forms are there then they not. Nobody can tell me what's happening .I'm getting desperate. Please help.
