Provident payout

I was dismissed from work and have send all the relevant documentation for my provident fund to be paid out. when I followed up they have informed me that the case is closed as they did not get hold of me and requested that I send a bank statement, identity copy. I have send it through and since then I have been struggling to get my monies. now they have called again stating that I have send the wrong documents and that I again have to send a bank statement, identity copy and affidavit why my banking details changed. I have put the bank account I am currently using when submitting the papers and now all of a sudden they are requesting an affidavit. I have been waiting for my payout for more than six months and everytime they are requesting more documents or stating that I need to call back after 2 weeks. can you please assist, so that the process can speed up
