Documents went missing

My late mother in-law passed away in 2012 and we app**** for the surplus metal funds. All the documents were correctly filled in by my late mother in-laws work and it was submitted in 2013. Since then I have been calling at least once a month and I was told they still busy with the process. Last year November 2014 /December 2014 I was told that the money has gone to SARS but it returned due to the dates that didn't correspond correctly and I must contact them back in January 2015. I called last week and I was told there is nothing. Today was the worse day because when I called, the agent never intro herself just telling me there is nothing then she cut the call. When I called back I spoke to Mrs. Ncebakazi and she gave me shocking news saying they can't find the documents and I need to fill in the form once again. I want to know what happend to the forms that we filled in before and how come the forms is missing, I believe there is ***** involve here and someone took that money or they destroyed the documents. It just can't go missing like that when I was told SARS returned the money because the dates were wrong. I demand this to be investagated. ***** is taking place here.
