UNTRUE LIFE COVER REJECTION REASON

Metroplitan declined a life cover stating(***) "Declined due to health condition identified on another application" I did NOT apply for any health cover before this one,, I demand they tell me they got this false information from, this impacts the future of my health policies, where did I apply for them to give me such a response?
