<p>I app**** for maternity benefits (UIF) in April 2016 and i was informed by the labour department that my previous employer "Metropolitan Health Group should send a UI19 form and a salary schedule which they sent but with an incorrect termination code, the incorrect code means i went to maternity in 2013 and never came back to work but i went back until i was retrenched in 2015. I have sent several emails and calls only to be placed on hold for a long time, the consultants at the Labour department have also called and sent emails they where also promissed that the correct UI19 form will be sent but till today nothing has been done.</p>
