I am a member of polmed with old membership no 153736.During October 2015 I app**** for x gracia and the application was forwarded to polmed from your Port Elizabeth office.i never received feedback and now have received letter of legal action from service provider.<br> <br> PLEASE INVESTIGATE WHAT HAPPENED TO APPLICATION.<br> <br> SERVICE PROVIDER:BAY RADIOLOGY<br> PR NO:***<br> DATES OF SERVICE:1,7,8,912 SEPT 2015 AND 14 OCTOBER 2015.
