APP**** FOR EX GRACIA -NO FEEDBACK SINCE OCT 2015
Good day.I am a member of polmed(membership no 153736).In October 2015 I handed in a ex gracia application which was sent in to polmed on the 15th oct 2015.I have not had any feedback and assumed that it was approved.Now I have received notices of legal action.<br> <br> Please investigate what happened to application.Application was for following:<br> <br> PR NO:***<br> DATE OF SERVICE:1 SEPT 2015,7 SEPT2015,8 SEPT 2015,9 SEPT 2015,12 SEPT 2015,AND 14 OCT 2015<br> SERVICE PROVIDER:BAYRADIOLOGY<br> <br> MR CAMPBELL<br>

Ex-Gratia approval was received on 15 February 2016 from the medical advisor.
The approval was send to the claims department for processing of the claims.
