Malpractice by Optometrist

The optometrist by the name Kayla at Mellins the Grove Mall gave me a script with incomplete test results. The CYL and Axis entries on my right eye were empty. She even added a clause at the end of the script saying she is not professionally liable. Your job as an optometrist is to do my test and give me the correct results. Not omit or change values because I am not making a purchase at the store. That is so *********. When I sent her an email to ask her about it she completely ignored me. I called and she wasn’t available. She didn’t even care to return a call. This is ********* and I am going to report her to HPCSA. It is completely unacceptable.
