UIF claim gone wrong

HI , I have been working for Ford for almost 13 years, the day I decided to leave, I sorted out my UIF claims and all that, it has been 4 months now , and I STILL CANNOT claim my UIF, because, no salary schedule forms have been given to me, when enquired, they keep sending me the UI-19 form, which is absolutely ridiculous , now I have heard that that salary schedule forms are now indeed compulsory when claiming UIF.

Why does this well known company NOT know of this ? Well its so called top of the range staff...

Very disappointed. I have worked my butt off for them, and no help.
