M-sure processed a claim without getting consent from me

M-sure processed a claim without getting consent from me. I called 3 times to raise the matter with them but on 2 accounts the call got disconnected and I was never called back. The dealer was liable to pay as the fault occurred within a few weeks of purchase and this was brought to their attention. Now the fault has come back now they claimed from the insurance and it got approved how? But the car is still within 6 months of purchase
