work done but no salary

In May 2014 I worked for the above mentioned company at Mpact New Era Springs for 3 days, working from 07:30 to 16:00 as the permanent sister was on sick leave. I worked as agreed, but I never got paid until this day. I was asked by Khethiwe to submit a signed timesheet which I did. 2 weeks later no money still. On an enquiry Khethiwe then told me to deal will Shiana regarding the payment. Shiana then sent me employment forms to fill out and send back, which I did. A month later still no salary. Suddenly Shiana tells me that I did not submit the required documents. I resent the employment forms for the 3rd time, then she told me that her boss is quiring the late submission of payment. I called after a month and again I did not submit. I am really sick of this. All I want is what I worked for.
