Terrible service!

I have never been so disgusted by service from a credit department of a hospital in my life. 4 times now I have asked to be give some sort of statement or invoice for my account and all requests, both phone and email, have fallen on deaf ears! And the kicker is being told that they don't have my email address yet on admittance I completed all my details on the forms. Yet they can send me an email requesting payment but apparently in order to send me requested information my email doesn't exist.
