Tax directive and IRP-5 Certificate

Good morning Lifehealthcare.

I visited Sars for enquiry and I was advised to request tax directive and IRP-5 Certificate for the amount R10404.00 provident fund. I was working for Lifehealthcare based a Waverley Care Centre. Upon termination of contract I didn't receive tax related documents.
