pmb and sickness forms filled out after amdission

Hi,

I was admitted in on the 31 May 2021 in casualty after a major car accident and was booked off for the week - 31 May - 6 June.

I require a doctor to please assist in signing my insurance sickness benefit forms. I contacted fourways life so many times but cannot get to the right personnel to handle this.

What I did not mention is I was engaging with someone from credibill and they ignored my mails for over 9 days.
