Nurse gambling with my child's life

I would like to lodge a complaint of misconduct or ***** committed by nurse/sister Matseko Mothibi. The nurse intentionally did not administer anti-biotic medication to my 2 yr old who is admitted in Life health Carstenhof. When I asked her colleague in the morning, as they were getting ready to leave in the morning the response was that the morning shift of nurses would administer the medication. No medication was administered to my sick child. I asked one of the Paediatricians what the instruction was for the antibiotics. Paediatrician advised 6 hourly was the instruction. I told the Dr what had happened to my child and the night staff, the Dr checked the file and said the nurse signed the file off as administered which wasn't the case. The paed asked one of the nurses to call her and confirm what happened. The colleague came back with feedback that nurse Matseko Mothibi administered the medication and she said "she took the baby whilst I was sleeping which is not correct. Please investigate as a matter of urgency.
