No one answers phones!!

I have recently received a summons from a lawyer stating I owe for blood tests which were done when I was taken to casualties at Glynwood hospital in Benoni back in 2013. I had a family member fill in the forms for me as I was not in a state to do so. It's quite apparent that something has gone wrong between the accounts department and the pathologists. I have had numerous calls between the medical aid (which I no longer belong to), the attorneys I've been handed over to as well as the pathologists. I have been told by the medical aid that I need to contact the Glynwood to get a copy of the forms that were filled in as well as the authorization numbers (whether approved or not). Good luck with that, I had no joy reaching the accounts department yesterday and today I was given a separate number to call, which also just rings. When I get through to the switchboard I am told by the lady answering (which through no fault of her own) does not know any email addresses that I can use to put my case forward. I am now sitting with a huge lawyers bill and I want to get to the bottom of this and sitting at the root of this is the forms filled in at the hospital and how they were processed
