Cannot get refund after 5 months of trying

Patient James Dobson ref 664948 We have been trying to get a refund from Life Healthcare for treatment completed on 12 Sept 2022. I have Credit note for the refund amount. Every time I have followed up, I am asked to complete yet another form that I had not been asked for before. I have completed 3 different claim forms and provided all documents requested. I have spoken to Liezel Steenekamp 4 times at Wilgeheuwel who says she has sent numerous emails to Tebogo Kompe in "Head Office". So far, I have not had even the decency of an acknowledgement or confirmation.
