IRREGULAR DEDUCTION

On the 8 September 2015 I app**** for membership and on the 15 October the first deduction was expected to go off and when it did not I alerted Liberty and they assured me that on the 15 November 2015 salary they will not deduct twice. On 26 October 2015 I had learnt from my employer that they had deducted twice an amount of R4682.00. On 26 October 2015 I filled in a request for termination of membership form and never withdrew it. I am now learning that on my November salary they have deducted the said amount being R2341 x 2 = R4682.00. I want the refund of the full amount deducted from my salary as I had terminated membership due their failure to honor our agreement. I cannot afford to pay so much for a medical aid in one month. In a telephone discussion with Fatima I indicated that I was terminating my membership. Please refer to your telephone recordings for ease of reference.
