Double debited for rewards own your life program

I am writing this on behalf of my wife, Prinal Appalsamy, policy number: ***. From commencement she signed up for the \own your life rewards\" program which is R49 per month however she is being debited R99 per month from commencment. She has tried querying with the call centre numerous times, even visiting the branch however all attempts were unsatisfactory. She now wants to formally request for all the over debited money be reimbursed to her and the rewards program to be cancelled with immediate effect"
