King Price refused to pay claim due to letterhead

My mom submitted a claim with Dignity Group and it was sent to the underwriter King Price. They requested a police report which we submitted but they wanted it on their letter head. We told them that we already had one with another letter head and the information that is important is what is on the police report and not the letterhead. Claim was never paid...it's a year later
