Jet Edgars airtime swim swap

My brother has a cellphone contract with Jet. Now in Dec I bought him a phone but it requires micro sim card which forces him to do swim swap. I have been making calls on his behalf since Dec 2017 asking them at the call centre to provide us with the forms that Vodacom require for him to do a swim swap. They have been promising to call me back or him regarding this but nothing. He has a phone he cant use because Jet doesn't give him the forms required to get the swim swap. It is now 3 months full they only thing we get is the reference numbers below:
