Charged for insurances and products on my account that was supposed to be cancelled!

I opened a Jet account in March at your store in Table Bay Mall, the cashier who helped me to open the account never asked or explained anything about insurances

I went back to asked her why I received SMS to say that a personal accident and dental insurance was enrolled as I haven't requested for this to be added to my account the lady in store said that I must just ignore the SMS and that it wasn't added to my account, when I phoned in to customer care the consultant I spoke with said that she would cancel all insurances and products from my account..The reference for this call is *** which was on the 25th of March When I requested a statement yesterday and checked it I noticed that I was charged for the insurances I asked them to cancel and the consultant promised that everything was canceled

Can someone please explain why this happened as I am very disappointed!!
