Intercare mall@55 doctor fails to assist me in colleagues absence

I consulted with one of they doctor. The pain medication he prescribed was not helping . I call in the following day and was told that the doc was not in. Told the lady my problem and she said she will call back, which she did and got an appointment for 10:30am with another doctor and I’m expected to pay this doctor again, I asked her why I pay again, she said this doctor must examine me and see what is the problem, cause she doesn’t have access to the other doctor report on me, I asked her can’t he/she call their colleague and she responded what if she can’t get hold of him. And the impression I’m getting here is that these doctors aren’t working together, He doesn’t work full time but the others can’t assist patients in his absence.
