Urgent Cancellation and Refund Request – Policy No. ***

I am extremely disappointed with the poor service regarding my insurance policy cancellation.

In January, I requested the cancellation of my policy and completed all the required cancellation documents. Despite this, deductions are still being made from my salary/payslip.

I request the following urgently:

• Immediate cancellation of the policy if it has not yet been processed • A full refund for the two months’ deductions made after my cancellation request • Interest on the deducted amounts due to the inconvenience caused

I also request written confirmation of the cancellation and refund process.

If this matter is not resolved promptly, I will escalate the complaint further through appropriate consumer protection and regulatory channels.
