Unpaid hospital claim

I my name is Fadia Mckar I have been with hollard hospital plan for many years now and recently my daughter was hospitalized and I put in a claim .I knew the moment they ask for so many documents that they didn't want to pay after submitting everything my claim still got denied. I want the money as they have paid every month and now they reject. With all sorts of reason.after being years in this insurance I feel they are ****mers and I want my money back or my claim paid out.i will also just cancel this plans as I have more than one.
