Sort out my ITC

Good Day , i took a funeral plan last year september and in the month of December my premiums were not paid , to my suprise now i wanted to apply for a loan and upon getting my ITC i noticed hollard is noted as unpaid for Dec 2014 and January 2015 . This has now affected my credit score and the bank declined my application. My premiums when they were not paid hollard went and took double so i dont understand why is this happening. Can someone in hollard fix this immediately , this is afuneral plan and it is not any long term insurance or some sort of a loan where you just decides to update someone ITC without even their knowledge . i need this fixed within 72 hours because you have now affected my family life .
