I'm frustrated

I opened an account with Jet stores 2017,only find out that hollard has open a policy for me without my consent. I'm trying to find out how did you open it, who did you call and where did you get my details and who is the beneficiary to that policy. Most importantly I want to close the policy and refund my money.
