Agent identity theft

I received my edgars account statement with the deduction of your policies of which i know nothing about I requested the documents telephonicaly and it was sent to me your agent Refilwe Moeketsane as stated on the papers did identity theft and also forged the signature it's not my signature and all 4 documents have different signature i didn't give consent and i don't even know where she got my I'd number and names and forge the signature of which is not even similar to mine I'm disappointed and want my money reversed back to my account
