Lost Phone

I lost my phone on the 07 March 2016, I went to game at centurion they told me to right an affidavit, after writing an affidavit I phoned Hollard they said they need case number. I went to game again and asked to cancel the insurance they asked me why I told them that I can not do up and down to the police station and they asked me why is Hollard wanting a case number because it was not ****** but lost. I want hollard to cancel both phone insurance that I have with them that is Jet and Game, I don't want them to debit my account this month.
