I am writing to urgently request the cancellation of an Income Protection Policy opened under my name, reference number ***. I do not know how this policy was opened, and I did not authorize the policy or provide authorization for any mandate or debit order associated with it. I therefore request that you investigate this matter urgently and cancel the policy with immediate effect. Please also provide me with written confirmation that the policy and any associated mandate have been cancelled and confirm whether any premiums have been deducted or are scheduled to be deducted from my account. I have attempted to contact your call center on several occasions without success. If this matter is not resolved urgently, I will have no alternative but to escalate my complaint to the relevant ombud. I expect your urgent attention and written feedback regarding this matter.
