Good day Pierre,

Thank you for your email. With reference to the above, we would like to make an appointment for you, however we require authorization from the Military Medical Fund it includes a copy A and copy B form. Furthermore, we can not guarantee that the Military Medicine Fund will approve surgery in one of the hospitals at which we operate. Once you have obtained an authorization from the fund, you can contact us for an appointment.

Note: I have attempted for almost 12months for somebody from GEPF to reply/respond yet get sent the same messages over and over,and nobody deals/has dealt with my request.
