10 weeks af requesting a form and still waiting - DISGUSTING!

My Godmother died on 1 July 2018. I notified GEPF on 12 July 2018. They indicated there was a funeral benefit etc to be paid. I requested the form to have this done on numerous occasions over the past 10 weeks - not 1 single reply. I phoned this morning and waited 30 minutes to talk to someone - when I spoke to her she was terribly upset by mu complaints over this shocking service and put me through to her Supervisopr Maria Mot??? who undertook to send me the forms. This was at 10 am - 3 hours later and I have received nothing - just the same as the disgusting lack of response since 12 July. What do I need to do to get these forms please!
