CLAIM FOR CASH PAID FOR DOCTORS

I am with Get savvi medical insurance and in order for me to have my claim approved I have to first send the bills through which is fine but now they have come back asking for me to fill out a reimbur*****t form <br> It must be stamped by the bank as well as a copy of your bank statement confirming the account number. This is clearly ridiculous as they are deducting a monthly premium for my policy why now do I have to provide this just for the claim to be paid out. No where in the contract does it state that this is the procedure if I pay cash and to claim my money.
