Debt review

I have been looking to apply for some account however always getting declined and I don’t know why, I have a MTN Account and payment is taken by direct debit, payment is always on time However I’ve recently downloaded the ClearScore app and it says I’ve missed 21 payments with the “Foschini Group” I don’t understand this. I was told to check if there’s any accounts under my name however I haven’t found any. I’m really disappointed as this is affecting me
