Blacklisted but had no clue

So I app**** for cell phone, but was declined and then I asked why. So they advised me to contact Experian,which I did an a week late they finally got back to me after security checks and they sent me my credit report.

To my surprise the following account seem to be in arrears.

Supplier:- Fodchini (RCS) High limit:- R0 Acc Num:- *** Acc Type:- Revolving Acc Status:- Open or current Status date:- 2015-02-25 Open Date:- 2013-10-24 Open Bal:- R12240 Current Bal:- R2172 Bal Date:- R2017-11-11 Last Payment:- R385 Last Pay Day:- 2015-11-06 Installment:- R385 Payment Status:- 9 months arrears Overdue Amount:- R2172

So the reason why I put in all this information is so that the Foschini group can tell me,where I opened up this account. Reason being is that when I checked my Credit report,it doesn't have any last and current address and for the fact that this account was opened at the time when I was staying at my previous address an I didn't receive any mail at that or at the address that is on my credit report.

Also this account is currently open an again I have had no contact from Foschini even though I have an email account as well.

Then it also states that the last payment that I made was in 2015-11-06,which I can't understand because at the time my mother was terminally ill and was hospitalized from October 2015 and passed away in January 2016. So me being in an out of hospital, where did I get the time because I had no extra time at that time.

So now my question to Foschini group is as follows 1. Where did I open the account? 2. At what store did I make my last payment? 3. Was it cash,card or eft? 4. What effort did you guy's put in to contact me? 5. What does prescribed debt mean?
